Reporting an Auto Accident

Accident occurs → Manager (direct supervisor) is notified immediately.

• If necessary or further assistance is required – please contact your regional manager.

Manager should get with Chad (chadleeson@gillstire.com) and locate nearest location to obtain a UA test (ideally this should happen the same day - within 24 hours of incident)

• Please do your best to execute this procedure, even if incident occurs on a Saturday – If you cannot reach your direct supervisor, please reach out to your regional manager

Manager should sit down with employee and complete an incident report (it is important that this is thoroughly completed with all party’s information) - (Ideally this should be completed same day)

• This report should be submitted to Kim Gilges within 48 hours of the incident (kimgilges@gillstire.com)
